Coaching is a vehicle to help you reach an even higher level of􏰆 well􏰇being and per􏰆formance in your health and in your li􏰆fe. As your coach􏰈, I will􏰉 partner with you in a thought-provoking and creative process that inspires and supports you to maximize your potential. 􏰉 I will assist you in connecting the dots between who you are and who you want to be. Functional Medicine Health 􏰁coaching is a whole systems approach that integrates the principles of Functional Medicine, functional nutrition, eating psychology, mind􏰇-body medicine, and positive psychology. It also considers your personal values, goals, work, balance, fulfillment, and life purpose. Through the coaching process, you gain clarity and valuable insights that enhance the quality of your life. We use reporting, exploring, and a consistent commitment to action to help move you forward. This proactive working relationship not only accelerates your progress but it supports you as you mobilize internal strengths and external resources 􏰆for sustainable change.

Did you know that 80% of people show tremendous improvement just by making real changes to their diet and lifestyle? You are, after all, the biggest driver of your health care outcomes! On the off chance that you are in the 20% who who needs additional testing and supplementation, your three month commitment to consistent and intentional focus on your health will help your practitioner be even more targeted with their clinical approach, yielding you the best results.

COACHING AGREEMENT

Expectations: I ask that you please be honest with yourself and with me throughout the coaching process. You can expect me to be straight forward, constructive and confidential. You can say anything to me, positive or negative in your responses to me, and let me know if something makes you uncomfortable or if you don’t want to respond to a question. The key to an effective coaching relationship is communication. Do let me know at any time if you have concerns that we haven’t addressed.

As your coach, I am a resource to be used to your best advantage. I will share concepts and insights and maybe redirect our session if needed to increase your success in attaining your health goals.
I expect your best. If you aren’t doing your best, I’ll ask you to. I know you’ve begun this process because you’re ready to grow. From time to time, I will make direct requests like, ‘Will you accomplish ‘ 􏰍􏰍􏰍􏰍􏰍􏰍􏰍X’ by the end of the month?” You always have the option of accepting a request, declining or counter􏰇-offering with something that might be more workable for you.

Reaching optimal health is a commitment to yourself. If I end up caring more about you than you care about yourself, this plan will not work. YOU are required to do the work to get well. Getting healthy can cost money, take time and it can be frustrating when things don’t happen as quickly as you expect. I am here to ask the powerful questions, encourage you to stick to your commitment to yourself and to provide you with support as you maneuver into a healthier lifestyle. Health coaching fosters sustainable change over time. It is not always a quick fix. It will take work and sticktoitiveness.

Confidentiality: I recognize that in the course of our work together, you may share with me the following􏰎: future plans, health, financial or 􏰂job information, goals, personal information, and other proprietary information. I will not at any time, either directly or indirectly, use any information for my own personal benefit, nor disclose or communicate in any way, information to any third party. I will not divulge that you and I are in a coaching relationship without your permission. I will hold everything that we say and do confidential unless you present as a danger to yourself or others. In this case, I will inform legal authorities so that protective measures can be taken. In addition, you should know that unlike a physician, or a lawyer, our confidentiality agreement is not protected by law. Should it ever happen, I cannot claim in court that I am unable to divulge the contents of our conversations.

Nature of the Relationship: You are aware that the coaching relationship is in no way to be construed as psychological counseling, psychotherapy or any type of medical intervention. In the event that you feel the need for professional counseling or psychotherapy, it is your responsibility to seek a licensed professional. 􏰁Coaches are not healthcare providers and they do not replace your licensed physician. 􏰁Coaching results are not guaranteed. You enter into coaching with the understanding that you are responsible for your own decisions and creating your own results. You are working with me on a volunteer basis for the purpose of creating a structure to identify and achieve your health and well􏰇being goals. Topics to be addressed may include mindset, nutrition, exercise, sleep, stress management, work goals, relationship goals, finding meaning and purpose and health challenges. We will leverage the importance of healthy lifestyle choices to help you achieve peak health.

Client Waiver: Simply stated, you understand that I am a Functional Medicine Certified Health Coach, certified by the Functional Medicine Coaching Academy in association with the Institute for Functional Medicine, who is offering motivational and educational services. I cannot be held liable for any advice, suggestions or guidance that I provide during our work together.

It is my intention that we co-create a partnership based on open-ended curiosity, respect and trust. This will allow you to freely explore the full range of elements that effect your well being. As your coach, I recognize that your challenges, concerns and emotions that affect your health and well-being are unique to you and I am committed to helping you design your own Personalized Health Plan to help you reach your goals.

I deeply believe in your capacity to create long lasting change. I will be your greatest champion, always encouraging you to try out new behaviors and grow beyond your comfort zone. In re􏰏flection, I will emphasize the learning opportunities present in each new experience.

The health coaching relationship is founded on a partnership of equals. The coach is the expert on the behavior change process and the client is the expert in their own life, values and health goals. 􏰐Being that it is a partnership, both the coach and the client have shared responsibilities and roles.

The Coach’s Role and Responsibilities:

  • To discover, clarify and align with what you want to achieve

  • To be responsible for the coaching process and to keep you moving forward

  • To create a supportive, non􏰇􏰂-judgmental environment where you feel free to take risks and learn from your successes and failures

  • Support your autonomy and promote self-efficacy as you generate solutions and strategies

  • To hold you accountable for the commitments you make to yourself

The Client’s Role and Responsibilities:

    • To determine the timeline for change

    • To set the best goal and action steps for change

    • 􏰌Executing the change

    • Assessing success, barriers, creating back up plans and additional action steps

    • To determine the focus of  all follow-up sessions

In this program, success is maximized for people who:

  •  Are ready for long term shifts and results

  •  Are ready to commit to themselves and to a healthier future

  •  Are willing to do the work involved to get well

  •  Are willing to show up with an open mind

  • Are ready to stop delegating the responsibility of their health to others

  • Are ready to master the mental game, turning intention into reality

  • Who understand that the implementation of the self-determined action steps will require a time investment to realize the desired changes – too busy will guarantee failure.

In this program, success is limited for people who:

    • Are married to their current lifestyle

    • Are unwilling to take an introspective look into their lives to see what’s working and what’s not and adjust accordingly.

    • Are uninterested in mindset shifts

    • Expect the coach to do all the work and those who want to just be told what to do

    • Are not coachable

    • Are not committed to the process

Refund Policy:

Most people see profound success within three months if they put in the work (see client’s responsibilities above), so that’s what I ask of you – three months – and if you still want out, I’ll refund the remainder of your annual payment or you can unsubscribe from the monthly membership. I do not offer refunds for completed months.
